Survodutide
Survodutide is an investigational dual glucagon/GLP-1 receptor agonist from Boehringer Ingelheim/Zealand Pharma that is not yet FDA-approved for any indication; Phase 3 obesity trials (SYNCHRONIZE program) have reported positive results, and the drug holds FDA Breakthrough Therapy designation for MASH, but no new drug application had been filed for obesity as of mid-to-late 2025 reporting.

What it does & how it works
Survodutide is a synthetic peptide that co-activates both the glucagon receptor and the GLP-1 receptor. GLP-1 receptor activation suppresses appetite, slows gastric emptying, and enhances glucose-dependent insulin secretion, while glucagon receptor activation is believed to increase hepatic fat oxidation and energy expenditure, an effect proposed to contribute to reductions in liver fat seen in trials. This dual-agonist mechanism is mechanistically similar to retatrutide's glucagon component but without added GIP receptor activity.
Typical results seen in research
In the Phase 3 SYNCHRONIZE-1 obesity trial, survodutide produced average weight loss of approximately 16.6% at 76 weeks compared with placebo, alongside reductions in visceral and liver fat. Companion trials in metabolic dysfunction-associated steatotic liver disease (SYNCHRONIZE-MASLD) and earlier Phase 2 work in MASH-related liver disease have also shown improvements in liver fat content and fibrosis markers, supporting its Breakthrough Therapy designation for MASH; obesity-indication results remain investigational pending regulatory review.
How it’s commonly used
In clinical trials, survodutide has been administered via subcutaneous injection using a prefilled pen, consistent with other peptide-based incretin/glucagon therapies. It is not currently FDA-approved or commercially available and remains accessible only through clinical trial enrollment.
