Orforglipron
Orforglipron is an oral, non-peptide small-molecule GLP-1 receptor agonist from Eli Lilly that received FDA approval under the brand name Foundayo on April 1, 2026 for chronic weight management in adults with obesity or overweight with weight-related conditions, making it the first GLP-1 pill approved without food or water timing restrictions.

What it does & how it works
Unlike semaglutide, tirzepatide, and other injectable incretin peptides, orforglipron is a small organic molecule (not a peptide) that binds the GLP-1 receptor at a distinct site to activate the same downstream G-protein/cAMP signaling pathway. This non-peptide structure allows it to survive oral administration and gastrointestinal absorption without the specialized absorption enhancers required by peptide-based oral GLP-1 drugs, while still triggering glucose-dependent insulin secretion, glucagon suppression, delayed gastric emptying, and hypothalamic appetite suppression characteristic of GLP-1 receptor activation.
Typical results seen in research
In the Phase 3 ATTAIN-1 trial, adults with obesity or overweight and weight-related conditions lost an average of approximately 12.4% of body weight (about 27.3 pounds) at the highest evaluated dose over the trial period, compared with about 2.2 pounds with placebo, along with improvements in cardiometabolic markers. Earlier Phase 2 data had shown weight loss and glycemic effects consistent with other GLP-1 receptor agonists, supporting its advancement to registrational Phase 3 trials.
How it’s commonly used
Orforglipron is taken orally as a tablet and, unlike some earlier oral GLP-1 products, can be taken at any time of day without specific food, water, or fasting restrictions, distributed in the U.S. in part through Eli Lilly's direct-to-patient LillyDirect channel with home delivery.
