FOXO4-DRI
FOXO4-DRI is a synthetic retro-inverso (D-amino-acid) peptide senolytic that disrupts the FOXO4-p53 protein interaction to selectively trigger apoptosis in senescent cells; evidence is currently confined to cell-culture and mouse studies (including naturally aged and progeroid mice), with no completed human clinical trials identified as of 2026.

What it does & how it works
FOXO4-DRI is a D-amino-acid (retro-inverso) peptide engineered to mimic the p53-interacting domain of FOXO4, a transcription factor that is upregulated in senescent cells and sequesters p53 in the nucleus to promote senescent-cell survival. By competitively binding the disordered p53 transactivation domain and displacing endogenous FOXO4, FOXO4-DRI frees nuclear p53 to relocate to mitochondria and trigger intrinsic apoptosis, largely selective for senescent cells because the mechanism depends on senescence-associated FOXO4 upregulation. Because this displacement mechanism spares most non-senescent cells, it is classified as a senolytic rather than a broadly cytotoxic agent.
Typical results seen in research
In cultured senescent human and mouse cells, FOXO4-DRI induces apoptosis while largely sparing proliferating, non-senescent cells; in mice, it has reduced senescence markers and improved measures such as renal function, fur density, and exercise capacity in chemotherapy-induced senescence models and in naturally aged or progeroid (XpdTTD) animals. These findings remain preclinical only — there is no published human clinical trial data on efficacy or safety, so translation to human aging or disease outcomes is unproven.
How it’s commonly used
FOXO4-DRI has been administered in animal studies by intraperitoneal injection and applied directly to cell cultures in vitro. It has no established human administration protocol and is not an approved or clinically studied human therapeutic.
