PT-141 (Bremelanotide): The One Peptide in This Category the FDA Actually Approved
Most research peptides never see a completed human trial. PT-141 is the exception — it's an FDA-approved drug called Vyleesi. Here's what its approval trials actually showed, including the modest effect size.
PT-141, also known as bremelanotide, is unusual in this space for a simple reason: it isn't just a research chemical. Since June 21, 2019, it has been an FDA-approved prescription drug, marketed as Vyleesi, for acquired, generalized hypoactive sexual desire disorder (HSDD) in premenopausal women — persistently low sexual desire that causes marked personal distress.
Mechanistically, bremelanotide is a melanocortin receptor agonist. Notably, the FDA's own approval materials are candid about the limits of that explanation: the drug activates melanocortin receptors, but the mechanism by which that improves sexual desire and related distress is described as unknown. That's a rare and useful admission to see in an approved drug label, and worth sitting with — regulatory approval didn't require a fully solved mechanism, just demonstrated efficacy and an acceptable safety profile.
The approval rested on two 24-week, randomized, double-blind, placebo-controlled trials enrolling 1,247 premenopausal women with HSDD, published as “Bremelanotide for the Treatment of Hypoactive Sexual Desire Disorder: Two Randomized Phase 3 Trials” (Obstetrics & Gynecology, 2019). The effect sizes were real but modest: about 25% of treated patients saw their sexual desire score rise by 1.2 points or more, versus 17% on placebo. On desire-related distress, about 35% of treated patients improved by at least one point, versus 31% on placebo. Patients in the trials typically used the drug two or three times a month, no more than once weekly.
Those numbers matter for calibrating expectations: this is a treatment with a statistically real but incremental benefit over placebo, not a dramatic on/off effect — which is typical for FDA-approved therapies in this category, and consistent with how the drug performs in practice.
The honest summary: PT-141 is the peptide in this list with the strongest human evidence, precisely because it went through the standard drug-approval pathway rather than staying a research chemical. That makes it a useful reference point for how different the evidence bar is between an approved drug and everything else covered on this site.
